I had this problem last summer with constant holes appearing in my lilly pads, but I couldn't find any insects or anything that seemed to be responsible.
The nice folks at Hill Country Water Gardens suggested it was some type of larvae and sent me to Lowe's for an organic product to mix and spray on the leaves. It only kills the larvae and doesn't hurt the fish or the plants. It is a bacillus and I can't find the container right now, but the folks at Lowe's who work with ponds, etc. should know what it is. You mix it with water, take the lilly out of the pond and spray both sides of the leaves, then put the lilly back in the pond. It also works on other plants that larvae attack. One application and things were much improved. I sprayed twice at the interval suggested on the packaging -- problem solved.
