Help! Why do my fish keep dying in pond?

We have an outdoor pond, lined with black plastic and cement built 8 yr ago. For the last 7.5 yrs we successfully breed 5 gold comets to 60 ! Yes there is
plenty of water plants, hyancinth, papyrus, lily, mondo grass in pot. Yes there is a pump , and the water seemed to be balance, and do not "smell".
Frogs comes and go, and some day there will be heaps of tadpoles.

One day, we work up and all the fishes were dead ! It was a horrible sight for my young daughters.

We have empty and cleaned every plant and rocks in the pond, with new pump and filters. Yet, we cannot even keep a fish alive overnight. What can possibly be the cause? Can anyone help me or point me to the right place to look for assistance ? Last week, I was shocked to see mosquito larvae in the pond, something that should never happened I thought !
